Responsibilities
Perform comprehensive assessments of high-risk patients
Develop, implement, and continuously reassess individualized care plans in collaboration with primary care providers
Complete admission, routine, PRN, and death visits
Coordinate care transitions and ensure appropriate levels of care
Collaborate with physicians, social workers, discharge planners, and other care team members
Arrange non-medical support services (e.g., housing, transportation) to promote compliance with treatment plans
Engage specialty resources as needed to optimize patient outcomes
Maintain accurate and detailed documentation of clinical, functional, and fiscal outcomes
Identify opportunities for health promotion and illness prevention
Proactively prevent adverse patient events and intervene promptly when prevention is not possible
Act as a patient advocate while protecting privacy and confidentiality
Qualification
Required
Associate's or Bachelor's Degree in Nursing or related field
Current, unrestricted Registered Nurse license (WV or Compact License required)
Case Management certification
Strong ability to analyze complex medical information and make sound clinical decisions
Preferred
Hospice experience required
Homecare Homebase (HCHB) experience required
Strong care coordination and communication skills
Ability to work independently in the field and manage multiple priorities
