Responsibilities
Develop, assess, and facilitate complex care management activities for members with primarily physical and behavioral health needs
Create personalized care plans and provide education to members and families
Conduct 5–7 member visits per week
Complete documentation within 24 hours of visits
Meet turnaround times, productivity metrics, and training requirements
Collaborate closely with interdisciplinary care teams and leadership
Qualification
Required
Master's degree as a Behavioral Health Professional
Active Ohio licensure (LSW required; also acceptable: LCSW, LMSW, LMFT, LMHC, LPC)
2–4 years of related experience in case management, home health, discharge planning, or behavioral health
Valid driver's license (field visits required)
Authorized to work in the U.S. without sponsorship
Attendance mandatory for the first 90 days
Preferred
Strong computer skills (Excel, Word, Outlook, Teams, OneNote, OneDrive, PowerPoint, browsers)
Works independently and manages change well
Critical thinking and strong communication skills
Comfortable with hybrid work: field-based visits + remote documentation
Personable, flexible, and team-oriented
