Admissions Coordinator Licensed

Responsibilities
Interfaces with referring entities/individuals either in person or via telephone and coordinate admissions information among interdisciplinary team (social services, business office, nursing)
Provides facility tours
Coordinates admissions information (including payment source) with social services, nursing, and business office
Gathers information regarding perspective admits and conducts preadmission assessment for appropriateness and need for placement
Obtains client’s medical information electronically or via facsimile
Data enters and distributes client information, admissions packet, etc
Works with Business Office to determine Medicare/insurance eligibility to track insurance authorization following initial approval. Provides forms to required staff, submits to insurance company upon completion
Works with Director of Nursing and physicians to determine provider status
Orders or ensures availability of necessary supplies i.e., low air loss mattress, enteral feeding supplies (formula, pump, tubing), ostomy supplies, CPM machine, CPAP, etc
Provides routine update to insurers based on information collected from therapy, nursing and social services
Documents insurance updates and notifies business office
Tracks reports for referrals not admitted and other reports as requested
Maintains/updates facility waiting list (LTC and TCU)
Wherever possible greets resident at time of arrival and assists to room. Orients resident to room, call light
Assists with Medical Records
Supports and communicates GA Advantage
Participates in weekly occupancy meetings with assisted living facilities
Maintains census standards, depending upon referral availability, to ensure facility meets budget/census standards
Qualification
Required
Currently licensed and in good standing as an LPN in the state of Minnesota
CPR certification required
Proficient computer skills required
Working knowledge of medical terminology and abbreviations
Ability to be on-call resource nurse and/or manager on duty rotation as needed
Able to maintain confidentiality
Detail oriented, accurate record keeping
Preferred
Experience in medical, long-term care setting or medical records experience preferred
