Responsibilities
Coordinate and manage individualized hospice plans of care
Deliver hands-on nursing care and emotional support to patients and families
Educate caregivers and guide them through the hospice journey
Collaborate with physicians, social workers, and chaplains to ensure holistic care
Respond promptly to changes in patient needs with expertise and compassion
Qualification
Required
Current Illinois Registered Nurse (RN) license
Graduate of an accredited nursing program
At least 1 year of medical/surgical, oncology, or hospice nursing experience
CPR certification (within 3 months of hire)
Strong clinical judgment and ability to work independently
Local residence with ability to respond to on-call within 30 minutes
Preferred
BSN
2+ years of medical/surgical, oncology, or hospice nursing experience
CHPN certification (or obtained within 2 years)
