InternCorewell Health is a healthcare organization seeking a Medication History Pharmacy Intern to support the medication history technician team. The role involves direct patient interaction to obtain and document medication history, under the supervision of a pharmacist, while participating in various pharmacy-related activities.

Responsibilities
Under the direct supervision of a Pharmacist, formulates, prepares, and delivers medications and pharmaceutical supplies
Exercises professional judgment in the interpretation of orders
Trained in medication reconciliation duties and pharmacy technician skills
Works with multiple medication automation systems
Interprets physician’s orders checking for correct dose, drug, route, rate and frequency of administration
Reviews patient profile for allergies, duplication of drugs and drug interactions and accurately enters orders into computer under the supervision of a pharmacist
Aseptically reconstitutes, filters, prepares and labels parenteral, sterile products, and chemotherapy, and dispenses other medications under the supervision of a pharmacist
Maintains documentation and record keeping
Participates in patient care activities such as drug use monitoring, pharmacokinetics dosing service and nutritional support services
Provides drug information to physicians, nurses, and other members of the team in collaboration with a pharmacist
Participates in special projects, such as data entry or research
Completes all Corewell Health assigned competencies by assigned due dates
Qualification
Required
Must be enrolled in pharmacy school with an anticipated PharmD graduation date of May 2028
Bachelor's Degree or equivalent Enrolled in accredited PharmD program required
LIC-Pharmacist Intern - STATE_MI State of Michigan required
Preferred
Previous hospital pharmacy experience preferred
