Case Manager - Part C Intake Coordinator (part-time)

NEOGOV is a provider of mental health, developmental disabilities, and substance use disorder services for children and adults in Chesterfield County. They are seeking a Case Manager to provide early intervention supports and intakes to children with developmental delays and their families, working within a multidisciplinary team.

Responsibilities

Provides early intervention supports and intakes to children 0-3 years old with developmental delays and their families, in family’s homes or the office
Participates in a team of multidisciplinary service providers to develop and implement treatment and service plans and monitor client and family progress
Possesses and utilizes thorough knowledge of child outcomes to guide parents on services for their child
Assesses clients using screening tools to evaluate and determine eligibility for services, program planning, and client and family needs
Conducts community outreach and awareness activities such as in-services and trainings; and
Performs other work as required

Required

Bachelor's degree in a human services field, such as psychology, sociology, rehabilitation counseling, or a related field
Two years of experience in a human services field, or an equivalent combination of training and experience
Completed Part C Certification Process as a qualified provider prior to assuming position
Pre-employment drug testing, FBI criminal background check and education/degree verification required
Working knowledge of case management methods, practices, and procedures
Ability to interview clients to identify deficiencies in personal, social, economic and health needs
Ability to work in a multi-disciplinary team environment providing treatment and rehabilitation services
Ability to communicate effectively orally and in writing
Ability to operate a personal computer and related software and other standard office equipment
Ability to develop and maintain effective working relationships with internal and external customers

Preferred

Experience in Part C local system
Experience in service coordination activities with young children with developmental disabilities
Extensive knowledge of Part C regulations
Individual planning and teaching skills
Ability to prepare complex records and reports
Proficiency with Microsoft Office
