Ennoble Care is a mobile primary care, palliative care, and hospice service provider seeking a full-time Care Coordinator, Embedded to join their team. The role focuses on providing on-site support within long-term care facilities, coordinating care for residents, and collaborating closely with clinical providers to ensure patient-centered support.

Responsibilities

Coordinate care for long-term care residents using Ennoble Care’s EMR
Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions
Collaborate closely with Ennoble Care providers, within the long-term care setting
Serve as a key liaison between providers, facility staff, residents, and families
Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner
Accurately document patient interactions and care coordination activities
Follow established workflows, documentation standards, and escalation protocols
Contribute positively to team collaboration and strong facility partnerships

Required

Legally authorized to work in the United States
Active, unrestricted clinical certification or licensure, such as: Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN), Or comparable credential
Minimum 1 year of experience in a long-term care, skilled nursing, or similar clinical setting
Comfortable communicating throughout the day via Microsoft Teams, email, phone, and in person
Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting
Strong organizational and time-management skills
Working knowledge of healthcare terminology and care coordination principles
Proficiency with Microsoft Outlook and Microsoft Office
Ability to work on-site, full-time, Monday through Friday (approximately 8:30 a.m. to 5:00 p.m. EST)
Valid, unencumbered driver's license
Reliable personal vehicle
Current auto insurance, with the ability to provide proof
Ability to pass a background check and drug screen
