Care Coordinator
Ennoble Care is a mobile primary care, palliative care, and hospice service provider dedicated to delivering high-quality care to patients in their homes. The Care Coordinator role is essential for supporting clinical operations through effective care coordination, documentation, and communication to ensure continuity of care across various settings.

Responsibilities
Coordinate care activities using Ennoble Care’s electronic medical record (EMR) system
Support providers and clinical teams through accurate documentation, follow-up tracking, and care coordination tasks
Collaborate closely with other Care Coordinators and internal care team members in an office-based environment
Communicate clinical updates, patient information, and care coordination needs to providers in a timely manner
Assist with care transitions, referrals, and coordination with external partners as needed
Maintain accurate, timely documentation in accordance with established workflows and standards
Follow escalation protocols and support efficient clinical operations
Contribute positively to team collaboration, workflow efficiency, and daily office operations
Qualification
Required
Legally authorized to work in the United States
Active, unrestricted clinical certification or licensure, such as: Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN), Or comparable credential
Must have minimum 1 year of experience in a clinical and administrative or care coordination role within a healthcare setting
Must have strong working knowledge of healthcare terminology and care coordination principles
Comfortable communicating throughout the day via Microsoft Teams, email, phone, and internal systems
Ability to cultivate and maintain positive, approachable working relationships while demonstrating a consistent, dependable presence
Strong organizational, time-management, and attention-to-detail skills
Proficiency with Microsoft Outlook and Microsoft Office
Ability to work on-site, full-time, Monday through Friday (8:30 a.m. to 5:00 p.m. CST)
Ability to pass a background check and drug screen
