Responsibilities
Build, maintain, and expand referral sources including regular meetings, follow-ups, and field visits to maintain strong referral pipelines
Represent the agency at networking events, health fairs, conferences, and community programs
Conduct outreach (in-person, phone, and email) to generate new leads
Develop and execute monthly outreach & marketing strategy
Educate families and referral partners on our services and care approach
Deliver presentations and educational materials about agency services
Create partnership marketing flyers, brochures, and outreach tools (with support from marketing team)
Enhance social media and community presence
Manage incoming referral pipeline from intake to admission
Ensure timely communication and follow-up with prospective clients and families
Collaborate with Intake & Staffing teams to ensure smooth onboarding & fast case openings
Track referral sources and conversion rates
Serve as a professional point of contact for referral partners
Maintain frequent check-ins & satisfaction follow-up
Address concerns and find solutions to strengthen referral relationships
Maintain CRM or referral tracking sheet
Report weekly/monthly on outreach activities, leads, and conversions
Track various related metrics
Qualification
Required
2+ years' experience in healthcare outreach or business development
Familiarity with home care programs (PCA/HHA, MLTC, Medicaid, Private Pay)
Strong relationship-building, communication, and presentation skills
Valid driver's license & willingness to travel locally
Strong organizational and follow-up skills
Preferred
Experience with Home Care or Healthcare Sales (preferred)
Bilingual
Knowledge of CRM or marketing software tools
