Build deeper, long-term relationships with fewer patients than in a traditional healthcare model
Provide truly comprehensive, holistic care as part of the value-based PACE program
Travel throughout California clinics
Qualification
Required
Licensed Nurse Practitioner or Physician Assistant
At least two years of experience working with the elderly
Must have prescriptive authority
DEA registration
Eligible to serve Medicare/Medi-Cal participants
Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter
Role requires travel throughout California clinics
Preferred
CPR and First Aid certification upon hire preferred
