Health Data Coordinator

Day Spring is a nonprofit organization in Louisville, Kentucky dedicated to supporting adults with intellectual and developmental disabilities. The Health Data Coordinator is responsible for managing health data, ensuring compliance with medical appointment paperwork, and accurately uploading resident data into the electronic health record system.

Responsibilities

Maintain clear, accurate records, as required
Monitor and update HRST Tracking Spreadsheet to assist with the continuity of scheduling medical appointments and for compliance monitoring
Compile reports and participant-specific documents, as directed by a case manager or direct manager
Ensure that the data entered in the online HRST portal matches the data entered in the organization’s internal EHR system
Collaborates with Compliance and Program team members
Ensure completion and receipt of medical appointment paperwork to ensure HRST and Oasis updates are completed within the specified time frame, per agency policy and state regulations
Send updated HRST notifications within a timely manner, including but not limited to case management agencies and DDID nurse for review
Provide appropriate correspondence to DDID nurse and auditors for clarification and information requests about HRST updated information
Provide leadership, support, and education to other staff members, as needed, regarding quality assurance and compliance needs about HRST education
Train on HRST education, as needed, and attend any additional HRST training, as needed
Monitor participant health trends and report to the Chief Compliance Officer
Perform an annual review of each participant’s HRST scores to ensure accuracy and ensure that all outside agencies receive HRSTs per regulations and requirements
Participate in the incident review process
Coordinate resident intake processes
Participate in the helpline on-call supervisory rotation (additional stipend received)
Obtain training to assist in providing direct care to residents, as needed
Assist with resident medical appointments, as needed
Attend team meetings and house meetings, as requested
Other duties as assigned

Required

At least 21 years of age
Degree in a related field and/or a minimum 1-year experience with adults with intellectual & developmental disabilities in a community setting
Previous experience working with the Supports for Community Living (SCL) waiver and related regulations
A good working knowledge of computers, online programs, and Microsoft Office applications (e.g., Excel, Word, SharePoint) is required
Must be able to work independently, take initiative, and be self-motivated
Must possess a valid driver's license to operate and drive a vehicle
Good communication skills (written and verbal) and organizational skills are a requirement
Critical and analytical skills required for tracking and monitoring changes in resident health
Demonstration of understanding and commitment to the philosophical tenets of Day Spring
