Serves as a link between the organization and healthcare providers, patients, and families by promoting Midland Care’s services and helping eligible patients access care
Focuses on building and maintaining relationships with referral sources such as hospitals, clinics, nursing facilities, and care coordinators, while educating them about hospice, home health, and other supportive services
Assists with referrals, supports parts of the admissions process, and helps ensure smooth transitions of care, often meeting with patients and families to explain services and answer questions
Participates in community outreach and educational efforts, represents Midland Care at events, and tracks referral activity, all while maintaining professionalism, confidentiality, and alignment with the organization’s mission
