This role supports value-based applications and payer agreements while fostering strong relationships with internal teams and CIN member organizations.

Responsibilities
Facilitate onboarding and ongoing activities for CIN participants in collaboration with the Physician Care Network team
Organize, maintain, and track network participation and payer agreements
Coordinate with payers and provider groups for population health initiatives and joint operating committee meetings
Support completion of Commercial, Government, and Medicare Advantage value-based applications
Develop and distribute population health reports to monitor cost and quality performance
Manage administrative tasks such as invoicing and scheduling meetings
Stay current on CMS changes and implement strategies for success
Ensure compliance with HIPAA and confidentiality standards
Qualification
Required
Associate degree required
Minimum 2 years in population health, health insurance, or a related field
Strong communication and relationship-building abilities
Analytical and data-driven decision-making skills
Ability to work independently and collaboratively in a fast-paced environment
Proficiency in spreadsheets, analytics tools, and project management software
Preferred
Bachelor's in Business, Finance, or Healthcare preferred
