Team Leader

Responsibilities

Ensures care and services are provided in a manner consistent with established standards of practice. Ensures care is individualized, optimizes the comfort and dignity of the patient and is consistent with patient/family needs and goals
Responsible for employing qualified staff and ensuring adequate orientation, education and evaluation of team members. Serves as mentor to team members. Makes field supervisory visits, pursuant to state and federal regulations, annually and as needed during the year to provide additional training, assess care delivery or monitor progress toward identified goals
Ensures that care delivered complies with agency policies and procedures and applicable federal and state hospice rules and regulations
Maintains fiscal responsibility by ensuring costs associated with patient care operations (including payroll, mileage, equipment, medications and supplies) are in line with budgetary constraints and organization policy. Ensures services provided are covered appropriately through the hospice benefit
Develops a culture of respect, compassion, teamwork and trust without losing sight of accountability to quality patient care
Works collaboratively with other Team Leaders and clinical leadership

Required
