Community Behavioral Health Worker- ACT
Integrated Services for Behavioral Health (ISBH) is a community-minded, forward-thinking behavioral health organization helping people along the road to health and well-being. The Community Behavioral Health Worker-ACT will support individuals with serious mental illness in achieving recovery goals through meaningful employment as part of a multidisciplinary team.

Responsibilities
Assist individuals in exploring their interests and goals related to work, education, and meaningful daily roles
Provide individual vocational assessments, job development, benefits counseling, job coaching, and ongoing follow-along supports
Cultivate relationships with local employers and community partners to identify job opportunities aligned with individual strengths and preferences
Educate clients on how employment affects benefits (SSI/SSDI, Medicaid, etc.) using evidence-based tools
Provide behavioral health and case management services in homes, workplaces, and community settings
Serve a shared caseload as a full member of the ACT team; participate in daily team meetings and treatment planning
Document all services accurately and promptly in accordance with agency standards and Medicaid billing guidelines
Meet productivity and fidelity requirements as defined by ISBH and DACTS
Participate in cross-training, utilization review, and quality improvement initiatives
Support program expansion into surrounding counties as needed
Qualification
Required
High School Diploma or GED required
Minimum of three years of relevant experience (e.g., behavioral health, employment services, vocational rehabilitation)
Valid driver's license, insurance, and reliable transportation required for community-based work
Demonstrated commitment to recovery principles and cultural humility
Knowledge of local community service systems and employer networks
Comfortable working in an Internet-based, mobile office environment
Able to engage and motivate individuals with complex behavioral health needs
Strong interpersonal, verbal, and written communication skills
Preferred
Bachelor's degree in a behavioral health-related field preferred
