Responsibilities
Incumbent is a professional social worker whose duties and responsibilities center on the care management of Veterans in the Community Living Center
The incumbent must use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans as they transition from the Community Living Center to appropriate placement alternatives
Care management responsibilities include the management, coordination, and provision of social work and supportive services to Veterans and/or their families
Has a high level of skill and expertise to establish and maintain effective therapeutic relationships with Veterans in the Community Living Center and/or their families
Completes thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families
Actively participates as a member of interdisciplinary treatment teams and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of a resident centered treatment plan
Acts as an advocate when it serves the best interest of the Veteran and family members/caregivers
Is available to provide consultation to other treatment team and staff members regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning, compliance with treatment, and discharge planning
Must possess skills and knowledge to provide case management services to Veterans and their families throughout the continuum of care. The incumbent will be able to appropriately utilize principles of human growth and development over the life span and will be able to assist Veterans in coping with the loss and grief experiences from disability and terminal illness
Enters all Veteran/family contacts in the electronic record using appropriate formats and templates. He/she is accurate, literate, and timely in charting, correspondence, and reports
Judges effectiveness of techniques and when indicated seeks advice from the clinical treatment team or adjusts practices if therapies seem ineffective
Serves on committees, work groups, and task forces at the facility as deemed appropriate by immediate supervisor
Provides coverage for other case managers in the facility as needed
If assigned to duty station in Sioux Falls, incumbent is required to participate in a rotation for on-call services to the emergency department
Performs other duties as assigned

Must be proficient in written and spoken English

United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE)
Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level

English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f)
For appointments at the GS-9 grade level, VHA social workers who are not licensed or certified at the time of appointment must become licensed or certified at the independent, master's level within three years of their appointment as a social worker
