Responsibilities
Independently performs validation of legal documents in alignment with department protocols and state statutory requirements
Scan legal documents in compliance with department and system protocols
Update patient and surrogate-decision maker demographics and authority in compliance with department protocols and state statutes
Assist providers, staff, patients, and other customers with Advance Care Planning and Surrogate Decision Maker requests and inquiries in alignment with department protocols
Collaborate to measure, evaluate and optimize outcomes to meet department and system strategic goals
Qualification
Required
Associate of Arts or equivalent combination of education/experience
2 years demonstrated knowledge/experience in: Working both independently and within a collaborative team while achieving goals; Utilizing organizational and communication skills; Building relationships with system teams to achieve specified outcomes
1 year demonstrated knowledge/experience with health-care related legal documents or related experience
Preferred
Knowledge of the purpose and process of Advance Care Planning
Knowledge of ACP process and resources within health care systems
Proficiency in Microsoft applications
Proficiency in Epic application
