This role focuses on providing high-quality primary care to homebound high-risk patients, managing chronic diseases, and ensuring patient-centered care to improve outcomes.

Responsibilities
Performing comprehensive in-home assessments for home limited/high risk patients
Diagnosing acute and chronic conditions, order and interpret diagnostic tests (labs, imaging, etc.)
Developing evidence-based treatment plans and prescribe medications within State scope of practice
Coordinating/Collaborating with care team, providers, specialists, and community agencies
Conducting serious Illness conversations including: POLST, Advance Care Planning, Portable DNR, Goals of Care
Providing acute care interventions as appropriate
Managing ongoing chronic disease care (CHF, COPD, CKD, etc.)
Providing health coaching and patient education on medication adherence, lifestyle changes, and disease prevention
Ensuring accurate clinical documentation of diagnosis to provide exceptional evidence-based care
Qualification
Required
Completion of an accredited graduate-level Advanced Practice Provider (APRN or PA) program
National Certification and New Hampshire State Licensure as a Nurse Practitioner or Physician Assistant
Preferred
A minimum of three years of previous outpatient primary care experience
