The Colorado Coalition for the Homeless is dedicated to preventing homelessness and creating lasting solutions for at-risk families and individuals. The Pharmacy Liaison role involves supporting pharmacy operations by assisting patients with medication pick-up, scheduling appointments, and managing documentation for reimbursement and assistance programs.

Responsibilities

Greet and communicate with patients in trauma-informed and respectful manner
Providing ready-to-pick-up medications to patients, including accessing the pharmacy operating system to determine if prescription is ready for pick up or delivery
Schedule follow up appointments for patients, as communicated from various integrated health team members
Assisting with packaging and processing medication orders for site delivery
Obtaining documentations and filing paperwork as directed by pharmacists
Record all clinical services in a timely and complete fashion in the EHR and RXQ (pharmacy operating system)
Maintain up to date records in EHR & RXQ
Record telephone and other clinical communications as needed in the EHR & RXQ
Manage tasks, PAQ and scanning as needed to have a complete and current chart
Maintain accurate information regarding client’s insurance status and complete CCARs/DACODs and other clinical authorization documentation as needed
Performs other duties as assigned

Required

High school or equivalent experience
6+ months of customer service experience
Consistently superb customer service skills; excellent interpersonal/assertive communications skills demonstrating a high degree of emotional intelligence
Knowledge of homeless issues and demonstrated sensitivity to and a desire to work with underserved populations
Valid driver's license required. This requirement may be waived, if necessary, based on overall candidate experience and current needs of the business
