Responsibilities

Schedule and confirm participant appointments while coordinating care services across the PACE team
Answer incoming calls and respond to participant and family inquiries in a timely and professional manner
Assist with care management tasks to support seamless service delivery for our participants

Qualification

Geriatric care experienceCPR certificationInterpersonal skillsCommunication skillsConfidentiality handling

Required

High school diploma/GED required
Experience in geriatric care or a strong interest in the field
Excellent interpersonal and communication skills
Ability to maintain confidentiality and handle sensitive information appropriately
Current CPR certification is required
