Program Coordinator (Per Diem/Temporary)
Community Care Cooperative (C3) is a non-profit organization focused on improving health and wellness through innovative programs. They are seeking a motivated Program Coordinator to support the implementation of programs addressing social health needs, gaining hands-on experience in program operations and community partnerships.

Responsibilities
Learn and become familiar with the rules, regulations, and requirements set forth by MassHealth for the Health-Related Social Needs (HRSN) Services program to ensure proper program delivery and compliance
Get to know and build relationships with HRSN provider partners, support the management of these partnerships, and learn about associated contracts and requirements
Support daily operations of Social Health programs, including assisting with referrals and reviewing service requests for members for the HRSN Nutrition and HRSN Healthy Homes programs
Learn and become proficient with the C3Engage platform to process referrals, manage service authorizations, and run reports as needed
Participate in other Social Health projects, including helping prepare for the 2026 SDOH Training series and supporting Health-Related Social Needs (HRSN) screening goals
Support the expansion of social health initiatives for Medicare members and out-of-state health centers through research and project support
Qualification
Required
Strong interest in public health and health care delivery and experience with federally qualified health centers (FQHCs)
Knowledge of the social drivers of health (SDOH) and how they impact health outcomes
Commitment to serving under-resourced communities
Self-starter with the ability to take initiative and work independently
Curiosity and enthusiasm for learning
Strong communication and research skills
Ability to collaborate effectively with internal teams and external partners
Attention to detail and comfort with basic data or process tasks
Proficient with Microsoft Office Suite, particularly Excel for basic data analysis and reporting, and PowerPoint for creating presentations
Proficient with digital communication and collaboration tools (e.g., email, Zoom)
Proficient writing skills, including the ability to create clear and concise training materials and documentation
Proficient in data entry and basic recordkeeping
Proficient customer service skills and ability to communicate clearly with diverse audiences
Currently enrolled in, or recently graduated from, a graduate program in public health, healthcare management, health services administration, or a related field
In compliance with Infection Control practices per Mass.gov recommendations, we require all employees to be vaccinated consistent with applicable law
Preferred
Experience with anti-racism activities, and/or lived experience with racism is highly preferred
