Assistant Team Leader
Postgraduate Center for Mental Health is seeking an Assistant Team Leader to assist the ACT Team Leader in overseeing multi-disciplinary clinical and case management services. The role focuses on engaging hard-to-reach consumers with serious mental illness and ensuring that program goals and objectives are met while providing community-based services.

Responsibilities
Assist with managing the day-to-day clinical operations and required productivity standards
Directs and coordinates consumer admission process and discharge process
Monitor and review Comprehensive Assessments, Service Plan Reviews and ensures required assessments are complete as needed
Reviews and ensures the submission of Incident Reports as required
Assists with conducting other required assessments with clients, such as Child and Adult Integrated Reporting System (CAIRS) reports and Assisted Outpatient Treatment (AOT) reports
Monitor high-risk cases and takes lead in case conferences, meeting and coordination of care
Sets standards of performance by overseeing all documentation to ensure quality, timeliness and accuracy, in compliance with Medicaid, NYS Office of Mental Health, NYC Department of Health and Mental Hygiene and agency standards
Adheres to ACT policies & procedures, agency policies and maintains appropriate professional standards
Conducts daily visits in the community, and subsequent progress notes and other required documentation in accordance with agency and OMH standards
Completes Wellness Plans, Comprehensive assessments and other documentation as required
Maintain a role as either a Substance Use Specialist, Family Specialist or Transition Specialist
Provide 24-hour crisis intervention on-call services on rotating basis
Performs other duties as required by ACT Team Leader
Qualification
Required
Master's Degree required
At least two years of experience working in human services with individuals with serious mental illness and those with serious mental illness and co-occurring substance use disorders
Preferred
LMHC, LMSW, LCSW, or equivalent in licensure in NYS preferred
