HIV Prevention Linkage Coordinator

NEOGOV is seeking a qualified person for the Clayton County Health District’s HIV Prevention Program. The HIV Prevention Linkage Coordinator will provide community HIV education, counseling, testing, and linkage to care, ensuring that those diagnosed with HIV receive the necessary support and services.

Responsibilities

Links HIV-positive individuals to care and re-engages HIV-positive individuals back into care with appropriate documentation
Utilizes the SENDSS Data to Care protocol to identify patients who are lost-to-care and are unable to be brought back into care by the clinic staff, and link them to care
Follows local protocol to identify strategies for engaging newly and previously diagnosed persons into care
Identifies and locates patients, newly diagnosed, previously diagnosed, and lost-to-care, to determine if and where they are engaged in care
Conducts community surveys to ascertain health needs, develop desirable health goals, and determine availability of professional health services
Identifies patient health needs, screens for barriers to care, provides education on HIV disease and treatment, makes referrals for needed services such as mental health services among others to increase and facilitate client self-sustainability
Provides HIV and STD disease intervention activities to include case interviews, partner counseling and referral services to get contacts/associates/suspects identified and brought for testing
Conducts HIV testing and counseling at various community and in-house events
Provides HIV education services for the Clayton County clients and community at large
Prepares and disseminates educational materials
Plans and provides educational opportunities for health personnel
Prepares and submits monthly service reports and quarterly narratives for the Health District and the State Office of HIV/AIDS
Maintains consistent communication, and coordinates services and activities with the HIV Coordinator
Meets with multi-disciplinary team on a scheduled basis
Participates on statewide Linkage Coordinators Quarterly Update Calls, including quality management meetings to collaborate, identify and report on issues such as barriers to care

Required

Bachelor's degree in education, social, behavioral, or biological sciences or a directly related field from an accredited college or university
one (1) year of experience in public health education or one (1) year of experience at the lower level Hlth Educator 1 (HCP030) or position equivalent

Preferred

First-hand understanding of issues related to living with HIV or AIDS
Experience providing case management services for people with HIV
Good working knowledge of best practice counseling approaches, interventions, and strategies that have been proven effective with target populations (men who have sex with men, transgender people, injecting drug users, the homeless population)
Possess a valid and unrestricted Georgia driver's license (within 30 days of hire if applicant is from out-of-state). Must maintain a valid GA driver license during employment
Experience in providing HIV education
Certified and experienced in HIV testing and counselling
Excellent interpersonal skills with the ability to relate to a diverse group of people
Excellent Communication skills both written and verbal
Proficiency in speaking and writing Spanish
