Mountain View Hospital is looking for a Discharge Planning Assistant to join their team. Qualification

Required

Associate degree or 2 additional years of experience in lieu of a degree, with high school diploma or equivalent
Basic computer knowledge
Effective written and oral skills
Ability to comprehend and follow detailed instructions
Ability to effectively present information and respond to questions from patients, staff members and community partners
Ability to speak clearly and obtain necessary information from patients, to listen and seek clarification when needed for accurate dispersal of information
Ability to organize, recognize and correct inefficiencies, handle and prioritize multiple requests
