The Hospice Operations Coordinator serves as the first point of contact for incoming inquiries and referrals, ensuring timely coordination of care placement and working closely with internal teams and physicians.

Responsibilities
Accept and process referrals from various sources with a focus on accuracy and completeness
Enter and manage patient episodes in the electronic medical record system
Notify physicians of accepted referrals and secure agreement for continued care in the community
Provide updates and data to the Manager for referral tracking and reporting
Communicate with clinical field staff to share relevant patient details and updates
Identify and address potential issues or challenges as they arise
Develop working knowledge of payer requirements including Medicare, commercial insurance, and Title 19
Coordinate with payers for authorizations and required documentation
Provide weekend intake and scheduling coverage as needed
Offer support to team members in other roles when possible or during high-demand periods
Engage in continuous quality improvement efforts and professional development
Qualification
Required
Strong ability to multitask in a fast-paced, service-oriented environment
Highly organized and detail-oriented with excellent prioritization skills
Professional phone manner and exceptional customer service skills
Proficient computer skills, with the ability to learn and navigate EMR systems efficiently
Comfortable working with diverse teams and cross-functional communication
Preferred
Clinical experience or familiarity with medical terminology
Familiarity with sales or intake processes is a plus
