Retention Care Coordinator
NEOGOV is seeking a Retention Care Coordinator to enhance collaborative re-entry efforts between various stakeholders and the community. The role involves developing and implementing strategies, providing case management, and advocating for justice-involved individuals to support their reintegration into society.

Responsibilities
Provide information about community re-entry related activities with a focus on social determinants of health
Research and evaluation re-entry initiatives, pilots, demonstration programs, and other RFPs to determine return on investment
Assist with project management of various related reentry initiatives on behalf of the Reentry Advisory Board (RAB), Stepping Up and other justice and re-entry related programs
Provide case management and supportive services for justice-involved individuals in alignment with the continuum of care strategy from pre to post release
Encourage and support resident engagement in post release treatment and programming through referral and linkage and robust collaboration with community-based providers
Assist in collecting and preparing monthly and quarterly data analysis of targeted re-entry and Stepping Up efforts
Represent OJPP and the RAB at various community and correctional resource fairs and expos throughout Ohio
Attend community events and conduct outreach to agencies, organizations and the faith-based community when deemed appropriate
Assist in identifying and building relationships with local re-entry, housing healthcare, behavioral health providers, Ohio Benefits Bank, and Veterans Justice Outreach to foster ongoing collaboration
Assist OJPP and the RAB in collaboratively advocating for housing, behavioral health treatment, job training, and employment opportunities for the formerly incarcerated
Answer calls courteously and professionally
Respond to written inquiries in a timely manner
Advocate on the behalf of the RAB and the formerly incarcerated
Interact with fellow employees, law enforcement, court, safety personnel, and representatives from non-profit, faith based and government agencies, as well as the general public
Attend trainings, webinars, conferences, and educational events to maintain proficiency in the field of re-entry
Maintain regular and predictable oversight of transitional and long-term housing programs for justice-involved individuals with a focus on those experiencing homelessness and severe mental illness
Coordinate the expansion of transitional housing options and linkages to recovery support services for justice-involved individuals
Collaborate with staff and community partners to engage individuals post-release in treatment and supportive services
Review weekly jail booking lists to identify frequent utilizers, conduct screening for housing eligibility and needs, and initiate referrals to housing partners to begin care coordination prior to release
Partner with community-based housing providers to develop and strengthen referral pathways from jail into safe and supportive housing
Establish and maintain partnerships with landlords willing to rent to justice-involved individuals and support housing navigation efforts
Facilitate access to case management, clinical services, peer support, and recovery resources to promote successful community reintegration
Track housing placements and retention outcomes while identifying service gaps and advocating for program expansion to meet the needs of individuals
Represent agency at various community and correctional resource fairs and expos
Attend community events and conduct public speaking outreach to local organizations when deemed appropriate
Identify and build relationships with local housing, reentry, healthcare, behavioral healthcare, and support service providers to foster ongoing collaboration and develop safe referral pathway from incarceration to safe and supportive shelter
Utilize motivational interviewing strategies to encourage participation of justice impacted populations in supportive services and programs
Provide a range of creative, innovative, and evidence based motivational interventions within group and one-on-one settings, to support and engage individuals, with a strong focus on achieving sustainable recovery and community re-integration
Provide evidence-based assessment, case management, intervention, and educations services to justice impacted residents
Provide linkage to appropriate community-based resources based on the participant’s risk factors and needs
Complete required charting and record-keeping in compliance with agency and funding requirements and privacy considerations
Participate in scheduled team meetings and perform other related duties as required
Qualification
Required
Bachelor's degree in human services, social work, or related field with three (3) years of experience in social services, case management, treatment and/or substance abuse services, or related experience; or any equivalent combination of training and experience
Must have an LSW, LPC, and/or similar certification
Must meet background check requirements
