Responsibilities
Support of admissions process by verifying all consents are signed and admissions processes are completed for each admission
Conducts client and family orientation with primary therapist
Provides case management services to include coordinating transportation needs, providing referral and aftercare provider contact, addressing outside provider needs, facilitating groups to support clinical services provided
Collaborate with referral sources for assessments throughout treatment as needed
Collaborate with treatment team to ensure client engagement in appointments/aftercare planning
Ensuring each client has an aftercare plan developed in collaboration with the treatment team/referral sources/funding sources
Other duties as assigned
Qualification
Required
Bachelor's degree in a human services related field; minimum of 2 years of post degree, direct care experience with population services
Bachelor's degree in a non-human services related field; minimum of 4 years of post degree, direct care experience with the population served
