Responsibilities
Perform comprehensive assessments of high-risk members
Collaborate with primary care providers to implement individualized care plans
Identify and continuously reassess appropriate and cost-effective levels of care
Coordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionals
Arrange non-medical support services (e.g., housing, transportation) to support treatment compliance
Engage specialty resources as needed to resolve complex care needs
Maintain detailed documentation of clinical, functional, and fiscal outcomes
Identify opportunities for health promotion and illness prevention
Prevent adverse patient events when possible and intervene promptly when necessary
Act as a patient advocate while ensuring privacy and confidentiality
Qualification
Required
Education: Associate's or Bachelor's Degree in Nursing (or related field)
Attendance: Commitment to mandatory attendance during the first 90 days
Active, unrestricted Registered Nurse license
Strong clinical assessment and care coordination skills
Ability to analyze complex medical information and make sound clinical decisions
Hospice and HCHB experience required
Preferred
Case Management certification
Experience in case management or community-based care
Home health experience
