Coordinated Access Network (CAN) Exit Coordinator

Responsibilities

Supporting the Exit Manager by reviewing vital documents in the HMIS database
Maintaining unsheltered program enrollments
Conducting criminal background checks to assist with the housing matching process
Reviewing verifications of disability and homelessness and following up with Case Managers as needed
Facilitating the Housing Case Conference Support Committee and Housing Case Conference Meetings in the absence of the Exit Manager
Assisting the CAN Landlord Engagement Specialist in the tracking, engagement, retention, and mitigation of landlords working with our system
Creating and maintaining a shared database of landlord partners and available units in our region and processing mitigation requests
Collecting housing applications, reviewing security deposit applications, and processing flexible fund requests from partner agencies
Supporting the Exit Manager in data entry in our shared database as well as cleanup for our By Name List, which is the active registry for all homeless households within greater New Haven
Collaborating with our marketing team to tell the story of United Way to ensure that our community understands the work and impact of ending homelessness in our community
Maintaining good working relationships with community, nonprofit, and business leaders in the community

Required

Bachelor's degree preferred with at least one year of similar experience; equivalent life experience may be substituted for education
Advanced proficiency in Microsoft Office applications, including Sharepoint, Outlook, Word, Excel, and PowerPoint
Excellent organizational skills and attention to detail
Strong communication skills, including basic business writing skills and a positive phone personality with active listening skills
Ability to work independently; adjusting to changing priorities
Attention to detail and strong organizational skills
Ability to establish and maintain high quality relationships with a variety of stakeholders
Ability to speak and write clearly and concisely for a broad audience
Ability to analyze and exercise sound judgment
Highly motivated and creative problem solver
Ability to relate to people of different economic and ethnic backgrounds
Ability to be diplomatic when working with staff from other agencies and clients in crisis
Valid driver's license and reliable transportation

Preferred

Experience working in the rental housing market
Experience working with various types of databases and content management systems
Knowledge of CT HMIS
Knowledge of the challenges and barriers that homeless people face
Bilingual, Spanish speaking
