Responsibilities
Oversees program enrollment
Monitors service coordination and performance measures
Inputs and reviews electronic documentation
Conducts screening and development of individualized reentry plans
Meets with reentry case manager and peer support team to assign clients and monitor client progress
Supports reentry case managers in implementing appropriate intervention strategies for the welfare of clients
Meets with service providers to assure services are being provided
Qualification
Required
Bachelor's Degree from an accredited college or university in Business or Public Administration, Social Service, Sociology, Psychology, or a closely related field
Two (2) years of responsible community development supervisory work
Must possess and maintain a valid Class C driver's license
Preferred
3+ years' experience working with justice-involved individuals or in a related setting
Experience using an electronic health record/database
Experience working with community-based agencies/organizations to access resources/services for returning citizens
Experience in program management
Licensed in at least one of the following areas: LCPC: Licensed Clinical Professional Counselor, LCADC: Licensed Clinical Alcohol and Drug Counselor, LCSW-C: Licensed Certified Social Worker-Clinical, LMSW: Licensed Masters Social Worker, LGPC: Licensed Graduate Professional Counselor, LGADC: Licensed Graduate Alcohol and Drug Counselor, LCSW: Licensed Certified Social Worker, LCP: Licensed Clinical Psychologist
