In this role, you will ensure the smooth delivery of high-quality home care services to clients by coordinating admissions, supervising aides, and maintaining client records.

Responsibilities
Coordinate client admissions, assessments, and care plans
Supervise and mentor Home Care Aides, conducting client visits and providing support
Facilitate daily operations meetings and address client and staff concerns
Maintain accurate client records and assist with payroll and billing
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
Build and maintain strong relationships with clients, families, and referral sources
Qualification
Required
Must possess a valid Pennsylvania Driver's License in good standing, free of traffic violations
Reliable, independent transportation
Must possess a High School Diploma or GED
Preferred
Scheduling experience preferred
