The FSC is responsible for the implementation and coordination of service delivery to CHP residents, focusing on serving vulnerable populations through various programming and services.

Responsibilities
Conduct individual and/or community assessments to determine the specific needs of each community and then intentionally connect residents with resources or services based on those needs
Establish partnerships and linkages with other service providers to develop and implement program activities focusing on Family Services, Housing Stability, and Senior Services
Work with residents to increase their quality of life by assisting them in reaching their individual goals and increasing their self-sufficiency skills
Provide a wide range of programming and services to youth, individuals, and families
Complete reports, enter data, and plan for monthly programs
Greet youth at the bus stop on their way to school and host learning lunches for those looking to apply for benefits and/or services
Qualification
Required
High School Diploma or equivalent, plus 2 years' experience working in the human services/social services field; OR
Bachelor's degree in the human services/social services field, plus 1 year of work experience in the human services/social services field, required
At least 1 year of experience must include experience working in an area of focus on delivering community-based programs
