Clinical Team Leader (Mental Health Services)

Vinfen is a nonprofit health and human services organization dedicated to providing community-based services to individuals with mental health conditions. The Clinical Team Leader will oversee the ACCS team, ensuring high-quality service delivery and managing transitions for Persons Served while acting as a liaison with external parties.

Responsibilities

Provide administrative leadership, supervision and direction of Integrated Team staff, including the GLE/SIE staff. Direct the overall work activities of all Integrated Team staff and all visiting/consulting personnel while working with the team
Ensure all Integrated treatment and supports adhere to the service goals of:
Sustained engagement and participation in services
Housing in independent settings
Community tenure without hospitalizations or other institutional placement
Greater symptom stabilization and self-management ability, leading to self-sufficiency of PS
PS satisfaction with services and personal outcomes
Successful transition to other DMH or community services
Function as the primary liaison with DMH, Behavioral Health Community Partners (BH CP), One Care and/or DMH Case Managers for care coordination integration
Hire, supervise and evaluate all Team staff, including the Residential Director. Orient, train and coach staff of multiple disciplines, evaluate all staff for role competencies yearly, access/design training opportunities for staff as needed to enhance competencies and job performance. Foster teamwork. Arrange new staff participation in his/her respective ACCS Community of Practice Workgroup
Monitor staff performance and productivity. Evaluate staff performance, including offering consistent and direct performance feedback by completing competency-based assessments, providing field supervision with Integrated Team staff, and directly observing staff while working with PS. Conduct corrective action procedures with staff if needed
Coordinate intake of all referrals to Integrated Team following DMH requirements. Assign staff to orient, engage and enroll new PS
Oversee service delivery to all PS, including participation in services, treatment and support skills planning and implementation of recovery goals, peer and family support, readiness for discharge, and support through service and life transitions, including acute care and housing transitions
Regularly review treatment plans to ensure treatment is effective in addressing the PS needs and goals that led to enrollment, barriers to movement, or additional needs identified while enrolled in ACCS
Ensure staff are trained in and use evidenced-based practices (EBP) including Motivational Interviewing; Harm Reduction and Addiction Treatment; Screening, Brief Intervention, Referral and Treatment (SBIRT); Housing First; Stages of Change; Seeking Safety, and Cognitive Behavioral interventions
Ensure staff are trained in and employ trauma informed, recovery-oriented, person-centered, culturally competent and strengths-based approaches and skills training techniques in their work
Ensure systems provide PS with Peer and Family Support, and promote the development or enhancement of natural supports and resources for PS. Work with Peer Specialists to reinforce and maintain a holistic perspective and recovery orientation for all PS
Maintain systems for input from Peers and families
Support the Assistant Team Leader in managing risk mitigations systems, including developing and maintaining risk alerts and procedures
Ensure the seamless integration of Team and GLEs/SIEs. Assign LPHAs direct responsibility for treatment plans and services to PS in GLEs/SIEs. Co-facilitate GLE/SIE Staff Meetings on a regular basis with Residential Director
Ensure adherence to Vinfen ACCS standards and EBP fidelity
Ensure the timely, thorough and thoughtful documentation of PS needs and services is provided
Ensure all staff receive role-based, certification- and licensure-specific supervision regularly
Maintain staff schedule. Manage staff time off to maintain operational excellence
Schedule LPHA Licensed staff to provide clinical coverage 24 hours/day, 7 days/week, 365 days/year, including on-call back up and on-site support and intervention when needed
Maintain team interventions and systems, and the external connections needed, to promote PS health, including the management of PS medications in compliance with Medication Administration Program (MAP)
Maintain team systems to coordinate employment services with Clubhouses, Massachusetts Rehabilitation Commission and other employment entities
Work with and direct the action of Integrated Team Consulting Psychologist and Occupational Therapist. Request consultation from Vinfen’s Clinical, Medical, Rehabilitation and Recovery senior leaders as needed
Conduct weekly staff meetings
Conduct Treatment Planning meetings as needed
Implement retention, recognition and awards initiatives for staff; provide guidance on career development and promotion opportunities to staff
Oversee utilization management and transitions of PS to other DMH or community services to promote service flow
Oversee DMH billing, contract and performance measures reporting protocols and compliance
Ensure adherence to company and program-specific policies and procedures
Design and oversee service data analysis, and quality improvement and management activities of the Integrated Team
Serve as resource and primary contact for neighbors, public officials, police and other service providers
Participate as member of Team Leader Community of Practice Workgroup led by the Psychiatric Rehabilitation Division’s Senior Vice President and Clinical Directors for continuous professional role competency development, mentoring and learning
May perform LPHA coverage duties when LPHA staff are absent
Participate in internal and external committees, meetings, and other activities as assigned

Required

Master's Degree, PhD or Psy D required along with licensure (LCSW, LICSW, LRC, LADC 1, OTR or RN) in Massachusetts required
Three years clinical experience in progressively responsible positions working with people with psychiatric disabilities and/or co-occurring disabilities required
Organizational, analytical, and communication skills
Ability to provide leadership to staff
Ability to work in a professional and confidential capacity
Knowledge of personal computer applications and equipment
Strong commitment to the right and ability of people served by ACCS to live, work, have meaningful relationships and receive the clinical treatment, resources and supports needed to thrive in their community of choice
Extensive knowledge of clinical treatment principles and modalities, best practices and evidence-based practices, including Motivational Interviewing; Harm Reduction and Addiction Treatment; Screening, Brief Intervention, Referral and Treatment (SBIRT); Housing First; Stages of Change; Seeking Safety, Wellness Recovery Action Plan (WRAP) and Cognitive Behavioral interventions
Extensive knowledge of recovery-oriented, person-centered and strengths-based values and principles and modalities, and knowledge of Peer Support principles and evidence-based practices, including the ethical standards of Certified Peer Specialists and Wellness Recovery Action Plans (WRAP), empowerment and self-advocacy techniques
Knowledge of the effects of prejudice including internalized negative attitudes about oneself, discrimination, and oppression of people with psychiatric disorders, and the effects of poverty
Extensive knowledge of trauma-informed and culturally competent services
Knowledge of community based education and employment services
All applicants must be at least 21 years of age and have a current, valid, US drivers' license for at least 6 months
