Responsibilities
Serve as project lead for the Rural Remote Patient Monitoring Initiative and primary point of contact for partners and stakeholders
Manage vendor selection, contracts, and implementation of continuous care technologies
Support hospitals and pilot participants with implementation, technical assistance, and workflow integration
Track program performance, outcomes, and data in collaboration with analytics and grant operations teams
Ensure compliance with grant objectives, reporting requirements, and funding timelines
Explore alternative payment models and reimbursement strategies to support long-term sustainability
Build strong relationships with health systems, vendors, clinics, and state agencies
Contribute to policy and legislative discussions related to rural health transformation
Assist hospitals in identifying and pursuing additional funding opportunities
Qualification
Required
Bachelor's degree
5+ years of experience working with hospitals, healthcare providers, nonprofits, or community-based organizations
3+ years leading complex initiatives involving multiple stakeholders, knowledge about clinical practice, and familiarity with efforts to improve community health and relationships
Strong project management, data analysis, and communication skills
Preferred
Master's degree in a health or policy-related field
Experience with grant writing and securing funding for healthcare or population health initiatives
