Responsibilities
Conducting outbound calls to high-risk members to build rapport and encourage proactive recovery interventions
Completing intake assessments and helping members develop care plans that prioritize their personal goals
Teaching empowerment skills and self-advocacy, helping members navigate everyday decision-making and resolve barriers to care
Identifying community resources (social, recreational, and clinical) and supporting members through the referral process
Participating in daily care team meetings and documenting all member interactions accurately in the electronic medical record
Qualification
Required
Must be a Certified Peer Support Specialist (CPSS) in the state of North Carolina
A willingness to appropriately share your personal recovery journey to support member engagement and instill hope
Approximately 3 years of experience in a related field (mental health/substance use disorder support)
High school diploma or GED equivalent
Strong communication skills for telephonic outreach and the ability to navigate electronic medical records
