Patient Care Coordination Program Assistant

Responsibilities

Provide transitions of care services to a select patient population, ensuring patients understand their follow-up care plans and required resources
Address barriers to care (e.g., transportation, equipment, food, appointments) by coordinating services and resources
Coordinate services across Michigan Medicine (Primary and Specialty Care), including communication with all care team members
Collaborate with external medical providers, institutions, and community partners (Community Mental Health, housing/shelter providers, food pantries, etc.)
Assist patients in obtaining necessary resources and overcoming obstacles to care
Help locate and obtain donated durable medical equipment (DME)
Identify patients needing higher-level care and coordinate referrals to additional providers as needed
Participate in case conferences, clinical in-services, and professional development opportunities
Review cases regularly with the Medical Director and/or Program Manager to assess progress and identify closure criteria
Maintain accurate, timely documentation in approved electronic systems
Collect and enter patient data, assist in tracking clinical outcomes and key metrics for program evaluation
Support administrative tasks such as maintaining and updating the program website, preparing reports, and organizing case management files
Provide project support work, such as flyers, communications, PowerPoint materials
Assist with scheduling, meeting coordination, and the distribution of program communications and resources

Required

Excellent interpersonal and communication skills
Ability to maintain confidentiality and professionalism at all times
Detail-oriented, organized, and committed to timely follow-through
Ability to work independently and as part of a diverse care team
Basic computer skills (Microsoft Office, Google Suite, EMR systems preferred)
Interest in healthcare, social work, public health, or a related field
Current driver's license and reliable transportation (preferred, but not required for all tasks)

Preferred

Prior experience in a health, social service, or community outreach setting
Familiarity with local community resources
Strong problem-solving and advocacy skills
Experience with website maintenance, basic web editing tools, or HTML (helpful, but not required)
MiChart experience
