Responsibilities
Provides day-to-day operations and oversight of post-acute liaisons, outreach coordinators, and patient management coordinators; covers vacancies or staffing shortages to ensure timely patient care and service continuity
Manages full cycle HR operations including hiring, staffing, performance management and employee coaching
Develops, reviews, and standardizes workflows, policies, and procedures for post-acute patient placement, referral management, and admission processes
Leads development of marketing and educational materials; conducts community outreach, rounding, and education to increase engagement through patient testimonials
Oversees data and analytics focused on admission conversions, challenges, referral metrics, and performance KPIs; drives service recovery and quality improvement
Assists with regulatory compliance, survey support, and standardized interfacility placement practices
Qualification
Required
Education: Bachelor's Degree or experience in lieu of degree
Experience: 4 years experience
License: Current & Valid Drivers License and Insurance and Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross
