Responsibilities

Demonstrates knowledge of and supports mission, vision, value statements, standards, policies and procedures, and the code of ethical behavior
Develop and manage technology enabled strategic initiatives that align with the state’s rural health transformation priorities
Lead the design and execution of rural health transformation technology enabled initiatives supporting care delivery, financial sustainability, and system integration
Build and maintain collaborative relationships with hospital and clinic leadership, provider networks, public health agencies, and payers
Facilitate cross-sector partnerships to integrate behavioral health, primary care, and social services within rural settings
Serve as a liaison between healthcare systems and community partners to promote data sharing, resource alignment, and collaborative impact
Convene cross-sector workgroups and facilitate community engagement and stakeholder meetings
Use product analytics and qualitative data to identify priority needs, measure effectiveness, and inform continuous improvement
Use quantitative and qualitative data to assess system performance, identify improvement opportunities, and measure impact
Develop and monitor metrics related to access, quality, cost, and sustainability
Prepare reports, presentations, and grant submissions for internal and external audiences
Manage budgets, timelines, contracts, and staff or consultant activities within assigned projects
Monitor federal and state policies impacting rural health delivery, reimbursement, and transformation programs
Ensure compliance with regulatory and funding requirements
Being present in the office is an essential function of the job

Required

Master's degree in business, public administration, public health, healthcare, finance, Information systems, or related field, plus two years of professional experience
Bachelor's degree in Public Health, Health Administration, Nursing, Business, Information systems or related field required, plus 5 years of progressive experience in healthcare system transformation, rural health, or health policy
Demonstrated experience in program design, grant management, and multi-sector collaboration
Strong understanding of rural health systems, community health improvement models, and health equity frameworks
Excellent communication, facilitation, and relationship-building skills across diverse stakeholder groups
Analytical and data interpretation skills to inform strategy and evaluation
Ability to manage multiple priorities in a fast-paced, collaborative environment
Establishing attainable metrics and methodologies to determine the success of agency programs
Knowledge of the administrative and legislative processes to represent the Agency
Being present in the office is an essential function of the job
