Health Records Coordinator

Responsibilities

Gathers and sorts medical information and documents received from paper sources and systems and prepares for scanning into the electronic health record
Creates digital images of paper records/documents, reviews images for quality and legibility, and stores images into the electronic record. Carries out basic calculations, and creates charts, graphs, and tables along with PowerPoint presentations
Analyzes requests for medical information, copies, and sends out in accordance with the facility and departmental policies and procedures. Assists patients, staff and affiliates with records requests for continuity of care purposes as well with other record related questions
Monitors record completion by reviewing, printing, and delivering Crystal reports noting incomplete or unfinalized assessments to the responsible clinical staff to ensure all assessments are completed and finalized within specified timeframes
Responds to requests and inquiries for patient health information and will request patient information as needed for the continuity of care, ensuring that all requests comply with all applicable organizational, local, state. and federal regulations including 42CFR Part 2. Will accurately retrieve and provide requested information while always maintaining patient confidentiality
Delivers outstanding customer service to both internal and external clients while presenting a professional and courteous demeanor. Responds to emails, answers phone calls, attends Teams meetings, takes accurate notes. Completes and maintains 100% on time required training
Performs other duties as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location

Required

At least two (2) years' experience in a clerical work
Knowledge of medical office procedures, spelling of medical terminology, punctuation use, and grammar
Knowledge and experience in the use of electronic health records
Knowledge of the maintenance and review of medical records for completeness, accuracy, and compliance
Knowledge and experience with state and federal guidelines regarding the release of protected health information
Experience in the use of personal computers and software applications such as Microsoft Word, Excel, and Outlook
Excellent verbal and written communication skills to effectively interact with various discipline levels and patients
Able to work independently, professionally, and productively with co-workers and all levels of staff
Must ensure the confidentiality of the clients and patients served
Detail oriented and thorough in completing work tasks
Able to cross-train in other areas within HIM, for example transcription, to ensure adequate coverage and support as needed
Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aid, Medication Aide and Employee Misconduct, List of Excluded Individuals/Entities (LEIE)
Males between the ages of 18-25 must be registered with the Selective Service
All applicants must be at least 18 years of age to be considered for employment at a state-operated facility. Waco Center for Youth applicants must be 21 years of age
Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours
Compliance with HHSC immunization policy and state hospital operating procedures related to immunizations is required
