The Patient Engagement Specialist will engage with patients and caregivers, facilitating their connection to Synapticure's care model and ensuring they receive the support they need during their initial outreach.

Responsibilities
Engaging eligible patients through telephonic, written, and digital outreach methods
Leveraging available data from EMR to learn more about potential patients and inform outreach processes
Completing the intake and consent process for eligible patients, and transitioning them to a Care Coordinator upon full enrollment
Engaging and fostering trust quickly with patients, acting as the face of Synapticure and the key point-of-contact to assist patients prior to full enrollment
Connecting authentically with patients and their families in a non-judgmental, respectful, and empathic manner and striving to meet their needs
Working daily within EMR to collect data, organize information, track tasks, and communicate with staff, patients, and family members; Providing feedback to the product development team, and, over time, becoming part of a super-user group to assist in onboarding and supporting others
Qualification
Required
0-2 years of customer service or sales experience with a successful track record to share for that time period
A passion for working within the community you are a part of, or have been a part of in the past
Dynamic interpersonal skills and a strong desire to improve the health of individuals and whole communities
A metrics-driven approach to your work and are willing to do whatever it takes to meet quotas for the Outreach team
Comfortable with high-volume outbound calls as well as creative outreach
An independent self-starter who responds well to change and shifting priorities
Excited about how technology can support your work and help drive the ongoing evaluation toward new, better care
