Responsibilities
Plans for and assigns responsibilities to staff responsible for providing community based and case management services
Coordinates service delivery with community partners including schools, medical providers and non-for-profit agencies
Completes medical practice compliance services
Completes chart and file audits
Ensures ongoing training of staff; interviews, hires and trains assigned staff
Identifies and resolves employee relations and performance issues
Provides appropriate leadership and direction and promotes positive morale, creativity and teamwork among staff
Evaluates employee performance and sets clear goals and expectations
Collects and compiles statistics and data related to various programs across Family Health Services
Prepares and monitors various reports on operations, billing and activities and presents finding to the Assistant Director of Family Health and the Director of PHS Clinical Operations
Responds to public inquiries and resolves complaints
Collaborates with other supervisory staff to develop department priorities and goals
Develops integrated procedures and practices to ensure consistency of service delivery
Monitors and analyzes program outcomes and implements quality control processes
Makes recommendations and initiates improvements to increase efficiency
Improve quality of services or result in other positive operational impacts
Keeps up to date on procedural changes impacting service delivery
Assists with the development and preparation of fiscally appropriate budgets that enable the department to meet its objectives
Monitors compliance with various funding sources
Develops and maintains working relationships and acts as liaison with external agencies and individuals who have a mutual interest in the department
Provides directions for case management functions and oversight of medical case management agencies
Participates in emergency response activities as assigned
Maintains required training, licensure and/or certification
Maintains confidentiality of privileged information and adheres to patient privacy laws
Demonstrates sensitivity and understanding of other ethnic groups and cultures
Available to respond to clients and/or customers, either in person or over the phone, during assigned business hours and/or clinic times
May provide clinical supervision according to program requirements
Demonstrates high ethical standards in the supervision of staff and provision of services to clients
Delivers high-quality services by anticipating and meeting the needs of both internal and external customers
Qualification
Required
Bachelor's Degree in Nursing, or related field
Five years of experience in public health which included at least two years of supervisory experience
Illinois licensure as a Registered Nurse
