Youth Services Support Program Coordinator
Alluvion Health is an FQHC committed to whole-person, whole-life care. The Youth Services Support Program Coordinator helps run the Youth & Family TREE Program, supporting adolescents and families impacted by substance use disorders and co-occurring mental health conditions, focusing on program coordination, data tracking, and community engagement.

Responsibilities
Coordinate daily program operations: scheduling, documentation support, and team communications
Serve as a liaison between participants and internal/external service providers to support access and warm handoffs
Collect, enter, and maintain accurate participant/program data to meet grant reporting and compliance requirements
Support referrals to other Alluvion Health services when appropriate (medical, dental, outreach/enrollment, prescription assistance)
Help plan, organize, and attend youth-focused events, coalition meetings, outreach/tabling, and community engagement activities
Assist with outreach materials, presentations, informational handouts, and public messaging (including social media support as assigned)
Support program quality improvement, audits, and grant-required fidelity standards
Maintain HIPAA compliance and confidentiality in all interactions
Perform other duties and special projects as assigned
Qualification
Required
3–5 years of related experience
Strong organizational skills and attention to detail (especially with data accuracy)
Strong communication skills and customer service mindset
Ability to collaborate with diverse teams and community partners
Proficiency with common office technology (Outlook, Word, Excel, internet tools); ability to learn an EMR/EHR
Valid Montana driver's license
Ability to pass a criminal background check
Preferred
Associate's or Bachelor's degree in Social Work (or related field)
Experience in a healthcare setting and/or FQHC
Certifications or experience such as Behavioral Health Aide, Mental Health Aide, Medical Assistant, Chronic Care professional, Certified Application Counselor, and/or SHIP
