Healthcare Program Coordinator (DOH/NMBHI #12941)
The New Mexico Department of Health is responsible for providing care to New Mexicans through various divisions. The Healthcare Program Coordinator will manage the admissions process, ensuring timely communication and accurate documentation for new patients seeking placement at Meadows.

Responsibilities
Screen all referrals received
Ensure that referral sources are made aware of the receipt of the referral and any missing documentation
Review admission packets with the Meadows team to determine appropriateness of admission
Meet with families to assist with the admission process
Remain in constant communication with internal and external parties regarding all admissions
Process new admissions into the electronic health record
Represent Meadows at public events as required
Qualification
Required
High school diploma or equivalent
Six (6) months experience with a public assistance program, medical, healthcare, educational, and/or state facility working with policies and procedures, conducting analysis/research, and/or providing or requesting information from the general public
Must possess and maintain a valid driver's license
Must obtain a defensive driving certificate
Preferred
Experience in skilled/long-term care
Experience in admissions processes and procedures
Experience in healthcare including the use of electronic records
Experience working as part of a multidisciplinary team
