Responsibilities

Responsible for program planning and coordination of wound ostomy service within Ohio County Hospital and Specialty Clinic
Oversee and develop wound care services in adherence to regulatory standards and physician orders
Lead educational efforts for hospital/clinical staff on wound/ostomy care topics, as well as educating patient and family members on wounds and wound care
Maintain continuous education and stay updated on the latest wound/ostomy care techniques and advancements
Committed to wound and ostomy care from prevention practices to intervention
Performs comprehensive initial and ongoing review of wound/ostomy patient care services provided, specialty equipment and service contracts, and supply utilization. Make recommendations regarding new policies and procedures to improve
This role strives to improve team effectiveness and system processes to improve patient outcomes across the continuum of care
Serves as a resource for clinical and hospital nursing staff for problem-solving and decision-making. Provides teaching to staff on both a formal and informal basis
Develop and implement innovations in patient and family centered care that fosters quality, safety, and effectiveness and serve as a model for care
Participates in decision-making, teaching, mentoring, and collaboration with the interprofessional team to advance the patient’s plan of care and achieve exceptional outcomes
Remains committed to wound and ostomy care from prevention practices to intervention according to evidenced-based wound/ostomy care for patients
Assists with the initiation and coordination of patient education
Provide training and education regarding skin/wound assessments, ostomy markings and assessments for new and established ostomies
Provide guidance regarding etiology and pressure injury staging
Support the patient care team in wound/skin breakdown-related prevention and treatment options
Develop standard operating procedures for the proper storage and maintenance of wound vac equipment and supplies
Develop standard operating procedures for ostomy care supply use, ordering
Determine list of required ostomy care supplies for medical surgical unit of Ohio County Hospital. Identify par level for supply management and outline proper storage in both patient care and non-patient care areas
Develop training & demonstration of ostomy markings and assessments for both new and established ostomies
Primary liaison between KCI rep and hospital/clinic staff
Coordinate with KCI to develop SOP for the use of wound vacs in both hospital and outpatient settings. Outline required paperwork, prior authorization, care coordination, and patient care congruent with contractual agreement and evidenced based practice
Assess storage of wound vac equipment and supplies for patient care. Develop standard operating procedures
Assess storage or ostomy care equipment and supplies for patient care. Develop standard operating procedures
Identify trends in nursing practice and establish plan with local leadership team to support quality patient care with pressure injury prevention
Participate in prevalence survey and ongoing surveillance of hospital acquired pressure injuries
Collect and analyze pressure injury data, identify expected outcomes, and provide input on the development and implementation of an individualized plan of care
Collaborate with clinical and support services and outside agencies or facilities in order to coordinate appropriate patient care
Make recommendations regarding new policies and procedures designed to improve the patient experience
Ensure that new policies and procedures are implemented in a timely manner

Required

EDUCATION: Completion of accredited program for registered nurse (RN)
LICENSE: KY RN or Compact State License
EXPERIENCE: 3+ years' experience in wound/ostomy nursing
OTHER: BLS

Preferred

EDUCATION: Bachelor of Nursing Degree (BSN)
CERTIFICATION: Professional certification in wound ostomy nursing highly preferred
EXPERIENCE: Experience with clinical quality, clinical operations, policy development and/or process improvement
SKILLS: The ideal candidate for this role has excellent verbal & written communication, demonstrate leadership skills, the knowledge of quality improvement processes, the ability to lead system change, and have experience building clinical networks. S/he will need to be able to effectively handle challenging situations, adapt to changes in work environment, and balance multiple priorities while working autonomously with minimal direction. S/he will have strong computer skills, including Microsoft Office, & Outlook. S/he should have experience with database entry, and the ability to analyze/utilize data, to support decisions
