Social Services Program Specialist-APS

NEOGOV is seeking a Social Services Program Specialist-APS to provide support through available social service programs to eligible clients. The role involves conducting assessments, coordinating services, and managing cases for vulnerable populations while adhering to state and federal regulations.

Responsibilities

Assesses senior adults in the community in response to referrals for abuse, neglect, self-neglect, or exploitation
Collaborates with law enforcement, first responders, health professionals, and social service organizations
Conducts in-person and phone interviews
Performs case management functions to include screening, interviews, needs assessments, case plan development, service coordination, and referrals
Files court documents
Assists customers in obtaining needed services
Creates thorough and detailed case management documentation
Maintains records and files
Establishes rapport with customers and resolves conflicts
Adheres to state guidelines for timely, appropriate responses
Represents agency and provides testimony in court hearings
Maintains confidentiality of protected client information
Processes eligibility for social services programs including but not limited to Adult Protective Services, NET Medicaid Transportation, Publicly Funded Child Care home provider licensing, and senior home repairs
Conducts in-person, phone, and electronic interviews
Requests verifications, contacts third parties, and reviews document submissions
Determines eligibility for program specific services and benefits in accordance with county, state, and federal guidelines
Conducts home visits to assess individuals and premises and recommends resulting actions
Monitors compliance for provider licensing requirements
Assists customers and responds to inquiries at the front reception desk, on the phone, and electronically
Coordinates with other agency divisions and local service providers to serve common customers
Makes referrals to community resources
Creates thorough and detailed case management documentation
Maintains records and files
Maintains current knowledge of program rules and changes through routine emails and trainings
Attends meetings and trainings relevant to programs and services provided
May represent agency at community events
Performs other related duties as assigned by the Supervisor, Division Administrator, and/or Director

Required

Completion of core course work for Associate Degree in Social Work or closely related field
18 months experience in Social Services or customer service in an office/administrative environment
Valid Ohio Driver's License
