Program Director (PACT)

Responsibilities
Serves as clinical and administrative liaison with the Division of Mental Health and Hospitals, the County Mental Health office, the Catholic Charities service network and other affiliated service providers. Works collaboratively to assure the development of a service delivery network responsive to the needs to the PACT consumer
Serves as a liaison with community service personnel and departments, i.e. police, ambulance fire, etc. to develop effective relationships, provide training and promote better understanding of the needs of the PACT consumer in an effort to de-stigmatize mental illness and promote more effective and sensitive response to the consumer’s needs
Responsible in conjunction with the specific PACT Team Program Leader for the development, supervision, recruitment, training and coordination of the PACT members and their professional activities thus fostering effective operation of the TEAM, committed to the goals of the PACT philosophy and responsive to the needs of the consumer
Responsible for the development, establishment and compliance with PACT and agency specific policy and procedures governing the delivery, coordination and operation of PACT services, including intake screening, emergency response, treatment planning, discharge planning, etc
Responsible for the convening, recording and coordination of PACT Administrative meetings, including Program Team Leaders and other members of each team as necessary
Coordinates and advocates with community service providers for ongoing, emergency and other forms of health care for PACT consumers, e.g. medical, substance abuse, nutritional, legal, etc
Oversees compliance with funding source, agency and other regulatory body requirements regarding the administration, fiscal and clinical service delivery expectations
Provides representation on behalf of the agency, the PACT team and/or the consumer at various medical, treatment planning and service coordination meetings
Performs other duties as may be deemed necessary and appropriate by the Associate Director of Nursing
Maintains appropriate clinical service data (MIS) in accordance with agency standards and reflecting specific regulatory body requirements
Responsible for the QI process governing the delivery, recording, evaluation and management of clinical, financial and administrative functions assuring compliance with established agency guidelines
Coordinates, in conjunction with the PACT Team Program Leaders(s) and/or provides PACT Team training and development in areas of clinical and/or management specialty
Provide direct and emergency back-up service support to the PACT Team Leaders
Oversee the establishment of direct and emergency back-up operations for 24-hour 7 day per week coverage
Conducts and/or directs quarterly service audits assuring the accurate and complete documentation of provided services in accordance with established policy guidelines for record keeping
Regular attendance and Participation in PACT Team Daily and/or Treatment Team Meetings, Case Reviews
Serves as ombudsman for consumer complaints or grievances
Participates in on-going supervision with the PACT Team Leaders, which may consist of field visits to the consumer's residence, work site, family, etc
Participates in the intake screening, treatment planning and discharge review process assuring proper management and decision-making processes are in effect with respect to all services available and offered to the consumer

Under the supervision of the Associate Director of Nursing, the PACT Program Director is responsible for the development, administration, coordination and delivery of consumer responsive community based mental health services to the assigned PACT consumers serviced through the County PACT Teams
The Program Director is responsible for the development and maintenance of the PACT philosophy and Team culture within the Teams and their respective personnel under his/her supervision, advancing the goal of servicing the consumer in the community through the flexible delivery of services aimed at preventing unnecessary or prolonged hospitalizations and reducing the unnecessary utilization of crisis and emergency services
The Program Director is responsible for the convening, recording and coordination of PACT Administrative meetings, including Program Team Leaders, and other members of each team as necessary to evaluate, promote and develop smooth and efficient communication and reporting systems between the PACT members, each PACT Team and to identify and develop problem solving strategies to address the needs of the consumer
The Program Director serves as liaison with the Division of Mental Health and Hospitals, the County Mental Health office, affiliated community service providers working collaboratively to facilitate the development of an effective continuum of care network for the PACT consumer
As a member of the PACT the Program Director participates in the planning and provision of interdisciplinary training and skill development for all members of the team
On-call response is required
