Clinical Case Manager - Villas on the Park (JR 5492)

The Clinical Case Manager will be part of the Site Based Housing Team, providing field-based outreach, case management, and support services to help clients secure and maintain housing.

Responsibilities
Perform initial comprehensive biopsychosocial assessment; develop a plan of intervention that addresses identified needs for housing, ongoing case management services, substance abuse treatment, mental health services, and coordination of needed community services
Collaborate with the client to develop an individualized treatment plan that addresses short- and long-term goals
Provide ongoing case management support to assess progress and ensure treatment plan outcomes are met or adjusted
Provide field-based therapeutic supportive case management services addressing mental health, functional, and substance use barriers as needed to help clients obtain and maintain housing
Consult with LCSW and supervisor to conduct crisis and suicide risk assessment and provide appropriate interventions, complete critical incident reporting as required per contract and PATH policies
Provide crisis intervention services focused on enhancing the clients’ ability to independently problem solve, utilize effective coping skills, and manage their own care
Qualification
Required
Master's Degree in Social Work or equivalent behavioral health degree eligible for licensure through the CA Board of Behavioral Sciences (e.g., MSW, MFT)
One (1) years of experience in healthcare, case management, homeless services or working with vulnerable populations
Have employment eligibility verification
Have or be able and willing to obtain CPR/First Aid training
Successfully complete the following as a condition of hire: Tuberculosis Test, Background Screening, Drug Test
Have reliable transportation
A valid driver's license
Proof of insurance and ownership for personal vehicles used during work duties
The ability to qualify for PATH's insurance coverage
Applicants must be authorized to work for any employer in the U.S
Preferred
Current and active licensure or registration with the California Board of Behavioral Sciences (e.g., ACSW, LCSW)
