Director of Community Programs
Healing Transitions is an organization focused on providing support for individuals facing homelessness and substance use disorders. The Director of Community Programs will lead and manage all non-residential services, ensuring effective program delivery and community partnerships while supervising staff and promoting a person-centered work culture.

Responsibilities
Provide direct supervision for Rapid Response (RR) and Unsheltered Homeless Response Pilot (UHRP) Team Supervisors
Provide guidance, and an innovative approach to service delivery. Support a positive, collaborative, and person-centered work culture
Support crisis management, problem-solving, and decision-making for program needs and guest/participant issues
Ensure all programs follow agency policies, licensing standards, funding regulations, and evidence-based best practices. Assist with budget development, resource allocation, and grant requirements
Represent Healing Transitions across multiple community multi-disciplinary teams and serve on relevant committees
Conduct presentations on Substance Use Disorders, Healing Transitions, and Community Programs
Serve as the primary point of contact/liaison for funders, collaborative partners, and other stake holders for all community-based programs
Ensure consistent, accurate, and thorough data collection and reporting
Utilize qualitative and quantitative data to inform and drive service delivery
Participate in audits, reporting and continuous quality improvement activities
Serve on senior Services Leadership team
Partners closely with Finance, Operations, Development, and other departments to ensure alignment, remove barriers, and support efficient, coordinated program operations
Participate in both internal and external multidisciplinary team meetings, case conferences, and staff training
Qualification
Required
Minimum of one year supervising Peer Support Specialist required, three years preferred
Must be technologically proficient across multiple applications, including Microsoft Office and database software
If a former recipient of any services at Healing Transitions, must have at least one year off campus, two years preferred
Preferred
Minimum of a Bachelor's Degree (Master's preferred) or 3 years experience in Human Services/Social Work or similar healthcare/social science field
1 year of experience working with the unhoused and/or substance use disorder population
Familiarity with relational databases - HMIS, Five CRM, and/or comparable database system(s)
Licensed Clinical Addictions Specialist, or comparable clinical licensure/certification preferred
