People Plan Program Manager

Government Administration

Responsibilities

Oversee the people plan case management program supporting residents through housing transition that seeks to support residents through all phases of housing transition, including eligibility verification, application completion, documentation gathering, and housing approval
Ensure individual assessments are conducted to identify barriers to housing approval (e.g., credit issues, criminal background, income verification, documentation gaps)
Ensure development individualized housing transition plans that reflect resident needs, preferences, and readiness
Oversees the program that assists residents with completing and submitting housing applications, vouchers, certifications, and required documentation
Coordinate with housing authority staff to monitor application status and resolve outstanding issues
Support residents in understanding housing requirements, timelines, and expectations
Track case management program output, outcomes, and program evaluation to ensure effectiveness of supporting residents through transition
Collaborate with housing navigators, case managers, social service providers, legal aid, and employment partners to address barriers impacting housing eligibility
Build and maintain relationships with landlords, property managers, and housing providers to expand placement options
Coordinate warm handoffs to supportive services such as employment, benefits, financial counseling, and behavioral health resources
Advocate in coordination with residents to ensure fair consideration and transparent decision making during the housing approval process
Facilitate resident meetings, workshops, and information sessions related to relocation, housing rights, and housing readiness
Provide trauma-informed, culturally responsive support during what may be a highly stressful transition period
Monitor program performance metrics including application completion rates, approval outcomes, and successful relocations
Prepare reports and briefings for leadership on progress, barriers, and emerging resident needs
Identify systemic challenges in the relocation and housing approval process and recommend policy or process improvements
Contribute to the refinement of relocation workflows, resident communication tools, and program guidelines
Research other city programs to learn from their effectiveness and process
Support evaluation efforts to ensure the Gilpin People Plan promotes long-term housing stability and resident well-being

Required

Bachelor's degree in social work, Public Administration, Human Services, Urban Studies, or a related field (or equivalent combination of education and experience)
Minimum of 3–5 years of experience in housing navigation, case management, resident services, or related human services work
Demonstrated experience partnering with public housing residents
Strong knowledge of housing eligibility requirements, housing vouchers or subsidized housing programs
Experience overseeing and evaluating community programs

Preferred

Experience supporting residents through large-scale public housing redevelopment or relocation efforts
Familiarity with trauma-informed care
Experience working with housing authorities, local government, or nonprofit housing organizations
