Responsibilities
Provide integrated whole person care evidence-informed interventions to the Learning Collaborative curriculum and TMaH practices. Share information on community, statewide, and national research relevant to whole person care, via learning sessions, webinars, or site visits. Provide information to coordinate resources referrals among all TMaH practices. Serve as a liaison between maternal health providers and other community-based providers. Serve as the lead TMaH staff in providing information to local, state and national organization on providing information on whole person care. Continuously be attuned to issues of health equity and share evidence-informed interventions to address health disparities. Learning collaborative sessions can be virtual, off-site, and/or require overnight or weekend travel
Uses quality improvement techniques with TMaH participants to incorporate screenings into their workflow and increase risk appropriate screenings, care and referral. Serves as technical expert in implementation of screening for perinatal depression, anxiety, tobacco use, substance use disorder, and HRSNs within the practice. Uses evidence informed practices to address health equity issues. Serve as a liaison between maternal health providers and other community-based providers. Works to establish referral pathways for TMaH practices and community service providers
Work within DHHS to advocate for policies, procedures, and financial provision of whole person care in perinatal health care. Analyze Medicaid screening data and referral data to identify utilization and trends. Prepare reports and advised management on finding and recommendations. Conduct annual survey on whole person care within each TMaH practice. Work with TMaH team to create value-based payment mode that supports whole person care
Meet with the TMaH participants to identify their current understanding and their preferences regarding implementation of whole person care into their office/practice routines. Works with TMaH practice to develop a plan for implementing whole person care. Provide practices with information/resources to allow them to provide whole person care in a manner that works for their practice, population, and business model. Directs and guides practices in implementation of activities to enhance whole person care across the workforce
Qualification
Required
A bachelor's degree and relevant program experience in nursing, social work, public health, or health care administration
Occasional overnight travel
Overtime and/or weekend work with Deputy approval
Valid driver's license
Lifting requirements: 20 lbs
Preferred
Master's degree in nursing, social work, public health, or health care administration
Relevant work experience in team-based care in a healthcare setting
Experience in maternal health care, public health, or related field with a focus on integrating mental health, substance use, and social determinants of health into routine care
Experience in training, coaching, and mentoring interdisciplinary teams
Understand value-based care principles and how they relate to patient outcomes
Excellent interpersonal, communication and facilitation skills with a demonstrated ability to build relationships with diverse teams and stakeholders
Ability to engage with and educate staff of all levels on the integration of whole person care into their practice
Ability to use data to assess performance and outcomes to drive continuous quality improvement
Experience with collaborative improvement initiatives in health care settings
